Woodland Sports  


Each student, grades 6-8, must obtain a medical examination prior to participation on a school-sponsored athletic team.    


The medical examination must be conducted by a physician, physician's assistant or advanced practice nurse chosen by the student’s parent/guardian at the student’s medical home (doctor's office). A complete sport physical packet must be returned to the school nurse on the date announced for the season or at least one week prior to tryouts.  Sport physicals are valid 365 days from the date of exam.   


If your student-athlete has a current sport physical, he/she may still be required to turn in the Health History Update Questionnaire. To check your student-athlete's requirements, feel free to email the school nurse at pbygott@barringtonschools.net, or call 856-547-8402 ext 231.


Forms may be picked up in the nurse's office or printed from the district website (district forms can be found on the "News and Information" tab).
